What to expect with your specialised care:

YOUR APPOINTMENT
You will need to complete a registration form and a health questionnaire at your first appointment.
Please bring the following to your appointment:
- Referral letter from a general practitioner or medical specialist
- Correspondence letter from your physiotherapist or podiatrist, or any other parties involved in the care of your presenting problem
- All relevant imaging and their reports (including x-rays, ultrasound, CT and MRI)
- Medicare card
- Private health insurance and TAC or WorkSafe claim details
The information from your registration form, health questionnaire and aforementioned documents will be recorded and used for the primary purpose of providing care for your presenting problem, including for administrative and billing purposes. It may also be disclosed to other health care professionals or involved parties with your consent.
Mr Wei-Han Tay will endeavour to see all new referrals within 2 weeks.
For urgent referrals, such as acute fractures or time critical injuries, please inform his staff who will discuss with Mr Tay – we will accommodate to ensure that your care is not compromised.
FEES
Consultation fees are to be paid at the time of your appointment by cash, EFTPOS or credit card (Visa, MasterCard or Amex). You will require a valid referral from a general or specialist medical practitioner to claim a Medicare rebate for your consultation. Additional fees may apply for any extra medical reports or forms that need to be completed.
Out-of-pocket fees for your operation are to be paid prior to your surgery by cash, EFTPOS or credit card (Visa, MasterCard or Amex). An itemised quote for your proposed procedure, including any out-of-pocket co-payments, can be provided as requested.
If you do not have private health insurance, you may choose to fund your own surgery with a payment plan as an option.


TAC AND WORKSAFE INJURIES
For patients who are awaiting approval from TAC or WorkSafe, consultation fees are to be paid at the time of your appointment and a receipt will be provided to you to claim back from the involved insurance company. Patients who have sustained an injury from a motor vehicle accident or at work, and who need operative management as a result of their injury, will require written approval from the involved insurance party prior to their surgery.
UNINSURED PATIENTS
For patients who do not have private health insurance and want to be managed through the public system, please ask your doctor to refer you to the outpatient clinic at the Royal Melbourne Hospital or Box Hill Hospital. Mr Tay will not be able to book you onto his public operating waiting list until you have been reviewed in his outpatient clinic at the Royal Melbourne Hospital or Box Hill Hospital.
